X-rays & Records
Please bring any dental appliances such as nightguards, dentures, and retainers for us to clean and/or evaluate.
If you would like your previous dentist to send x-rays or records to our office before your visit, they can send them via email. If you have not been to the dentist for a long time or do not have access to your records, no worries!
We can easily retake any x-rays for you at the time of your appointment.
In some cases, having past records can help our providers gain an understanding of a complex dental history or avoid duplicate x-rays – but it is by no means necessary for care.

Insurance
We will help clarify what your plan covers and what you may be responsible for. As a courtesy, we communicate with insurance company on your behalf to eliminate the hassle, allowing you to focus on improving your oral health.
Dr. Daniel Ramirez accepts Denti-Cal & Medi-Cal. Dr. Ramirez is a premier Medi-cal Dentist.
We accept most major PPO dental plans. Dental insurance coverage varies significantly from patient to patient, Please give us a quick call or send us an email for specific details of your plan and we will be happy to do a complimentary benefits check for you!
If you have an insurance card please bring with you to your first appointment or if there are any changes in your plan. In some cases, you may have to contact your HR department for more details on your insurance plan. (Note: Some plans do not issue insurance cards and use your social security number instead).

Name of insurance provider
Employer
Subscriber date of birth
Subscriber social security number
Relationship to subscriber
Group / policy number
Insurance estimates will be provided before any treatment is initiated, keep in mind, that in some cases, Insurances may decide to contribute less than originally estimated. In order to treatment estimates to be as accurate as possible, changes in insurance information should be reported to us immediately. Client will be responsible for remaining balance.
